Fill out the form on this page.
Call HME at 800.848.4468, select option 1, 2, 3 to get an RMA number.
(Return Material Authorization. It is important to put the RMA number on the return label and on the box.)
Place equipment in a box and place return label on box.
(Including packaging around your equipment for protection during shipment.)
Drop the box off at any authorized UPS service location or give it to your UPS driver.
Notes: For your convenience, the label will also include a link to the UPS Drop Locator. Please keep a record of your UPS tracking number for future reference.
